End of life care providers
If you need end of life care, you may think that you have to move away from home. However, a lot of care services can be provided to you in your home by the community palliative team, or as a day patient at a hospice or hospital.
If you are in hospital, your care will be provided by the hospital doctors and nurses, as well as the hospital palliative care team.
Depending on your needs, other health and social care professionals may be involved in your end of life care, such as:
- your GP
- community nurses
- counsellors
- our social care team
- chaplains (of all faiths or none)
- physiotherapists
- occupational therapists
- complementary therapists
End of life care at home
There are many health and social care professionals who can provide you with end of life support in your own home.
Macmillan Community Nurses
Macmillan Community Nurses support anyone who has a life-limiting disease within West Northamptonshire.
They work with other health care professionals to provide patient-focused, responsive, timely and sensitively delivered palliative and end of life care.

Marie Curie
Marie Curie is a registered charitable organisation that provides care and support to people with terminal illness.
In West Northamptonshire, they:
- commission a range of providers to deliver palliative and end of life care to our residents
- offer you expert care, guidance and support to help you to get the most from the time you have left
- have dedicated and caring nurses who work night and day in people’s homes across the UK, providing hands on care and vital emotional support
- have trained ‘helper volunteers’ who can visit you for a cuppa, and be there for you when you want to talk
- are there to support you throughout your illness with practical information and support on all aspects of life with a terminal illness

Cynthia Spencer Hospice provides local end of life care, including palliative care, to residents of West Northants.
